POSITION IDENTIFICATION
Functional Title: Case Manager-1
Program: WoRC Program
Supervisor: Case Manager Supervisor

Status: Non-Exempt
POSITION SUMMARY
This position is responsible for assisting program participants in becoming self-sufficient through assessing participant’s strengths and needs, assisting them in removing barriers so they may obtain job placement and job retention. Also responsible for maintaining all records and ensuring all records are in order.
ESSENTIAL JOB FUNCTIONS
Must have manual dexterity to type and use a computer. Must be a team player to ensure overall program success. Strong organizational skills are required. Must have strong communication skills, both verbal and written. Must be able to drive a car and travel locally and to out-lying communities. Must possess a valid Montana Driver’s License, proof of personal automobile insurance, must meet insurability requirements for agency automobile insurance policy. While performing the duties of this job, the employee is regularly required to talk or listen. The employee frequently is required to stand and sit. The employee must occasionally lift and/or move up to 25 pounds. Must have the ability to work with low-income and disadvantaged people with an objective, non-judgmental attitude.
DUTIES AND RESPONSIBILITIES
1. Assess participant strengths and needs to develop employability plans to aid participants in achieving self-sufficiency. Maintain participant files and required client status documentation.
2. Monitor and document participant progress to evaluate and modify employability plans as necessary via appropriate computer system.
3. Identify the need for intervention and refer participants to appropriate resources, assist with SSI applications, monitor day care needs and support services as necessary.
4. Provide orientations, program overviews, assessments, employability planning, good cause meetings, and budgeting.
5. Monitor participant’s attendance in scheduled activities on a daily/weekly basis via attendance sheets, weekly activity sheets, and employer contact forms and wage statements
6. Facilitate group activities that are part of program curriculum.

7. Meet with participants to review progress and assist with problem solving.
8. Participate in staff meetings; review individual cases that present unusual and difficult circumstances.
9. Communicate and coordinate with other service providers in order to provide quality customer service and avoid duplication of services.
10. When appropriate refer participants to placement in worksites based on skills, experience, and job readiness.
11. Perform other duties as may be assigned from time to time.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE:
Bachelor's degree (B. A.) in Human Services or related field, or High School Diploma or GED and at least four years experience in related human services field, or equivalent combination of education and experience.
OTHER SKILLS and ABILITIES:
Prefer knowledge of local labor market information and employer expectations.
