Demi Moore has checked into an addiction treatment center in Sundance, Utah, according to a report.

E! News says that the embattled star has checked into the Cirque Lodge, an upscale facility whose past clients have reportedly included Lindsay Lohan, Eva Mendes and Mary-Kate Olsen.

ET has footage of a 19-year-old Demi Moore as she celebrated a birthday with her General Hospital co-stars in 1982 featuring wild, tipsy behavior from the star in the interview.
